Description of intervention(s) / exposure
Patients/carers will be randomly selected by Curtin university social work students. The students will conduct interviews in pairs and transcribe the patient/carers experiences.
The purpose of the interviews are to record the patients and/or carer's perceptions and experiences on each stage of the health continuum, from the emergency department to RITH.
Interviews will take approximately 1.5 hour of a patient/carers time conducted on 1 visit. It is aimed for all interviews to be completed within a one month period.
Patients/carers will also be asked if tape recording or video recording can be conducted. We would like to capture video footage to use at conference presentation though realise that this may have an impact on the quality/honesty of the patient/carers story so this will only be conducted on those patients/carers where the story is a powerful learning experience and where that patient/carer agrees to this occurring.

Key inclusion criteria
To capture a variety of patients/carers journeys we aim to have representation from a number of patient/carer groups including: Those referred directly from an ED; Those referred after a long period of time as an inpatient >10 days from our major referring specialties being general medical, orthopaedic rehabilitation, stroke/neurology and stroke rehabilitation; Those referred after a short period of time as an inpatient <10 days from our major referring specialties being general medical, orthopaedic rehabilitation, stroke/neurology and stroke rehabilitation.

Patient’s/carers will be excluded based on the patient’s ability to participate such as: Moderate to severe dysphasia; Moderate to severe dementia; Inability to communicate in more than one domain (ie. written and spoken); Significant mental health presentation; Unsafe home environment for research staff.; Elective surgery

To follow a patient’s journey via interviews through the hospital system from presentation to ED through to discharge from the hospital service where RITH has been involved in the patient’s rehabilitation. This will provide the RITH team with: an understanding of exactly where patient and carer experience can be improved; 'mobilising narratives' that will help inspire RITH to improve the service they provide; a teaching tool to provide to referrers regarding the benefits of hospital substitution and choosing the right patient cohort.
