Usual rehabilitation (both in-patient and, if required, out-patient) will not change as a result of this study. This includes comprehensive discharge planning by the multidisciplinary team. Follow-up will occur 1-2 months after discharge from the rehabilitation facility.

All patients discharged home from the Stroke or Specialist Rehabilitation Units at Hampstead Rehabilitation Centre or St Margaret's Rehabilitation Hospital.
