Intervention participants will receive:

1. Health literature, and
2. An active referral to the NSW Get Healthy Coaching and Information Service

1. Health Literature:
Intervention participants will receive health literature brochures including information on behavioural risks and providing guidance on behaviour change. These brochures were designed by the research team in conjunction with the Mental Health Service for a previous research study.

2. Intervention participants will receive a proactive referral to the NSW Get Healthy Coaching and Information Service: https://www.gethealthynsw.com.au

The NSW Get Healthy Coaching and Information Service is a free, government funded telephone support service that supports people to make healthy lifestyle changes. The service aims to take clients through a 6-month coaching program, providing information on healthy eating, physical activity, healthy weight and weight management, and type-II diabetes prevention. Following the referral, a Healthy Lifestyle Coach from the service will make contact with the participant. Participants can elect to receive ‘information only’ (a single phone call and hard copy resources mailed from the service) or may opt to enrol in the 6-month coaching program.

Coaching program: Participants who choose to commence the program receive up to 10 calls over 6 months, during which they are provided with advice and support to achieve health behaviour change. Calls are delivered by university qualified health coaches (dietitians, exercise physiologists, nurses), who are trained in motivational interviewing. Coaches assist the clients to set their own behavioural goals and provide additional tips and educational materials. During their first call with a coach, participants complete a health assessment with information collected around their current health behaviours, health-related goals, and any current or previous mental health conditions. Participants are allocated to a coaching module (including: the standard program, Aboriginal and Torres Strait Islander cultural adaptation, and diabetes prevention program) based on responses to the health assessment. Each module aims to support health behaviour change. It may also focus on particular health concerns including alcohol reduction, type 2 diabetes prevention or may include tailored modules for specific populations including pregnant women or those who identify as Aboriginal and Torres Strait Islander. Participants enrolled in these specialised programs can receive up to 13 calls.

Key inclusion criteria
Participants will be recruited from mental health services across several local health districts and community managed organisations in New South Wales, Australia.

To be eligible, participants must:
- Have a working telephone number
- Currently be currently receiving care or have received care in the past 12 months for a mental health condition from one of the participating mental health services or community managed organisations
- Upon contact, confirm a willingness to engage with or be contacted by a telephone-based support service.

Key exclusion criteria
Clients who are under the age of 18 or identified by mental health staff to be too unwell or clinically inappropriate to contact will be ineligible for inclusion. Clients who self-report being currently enrolled in the Get Healthy service coaching program or having used the service in the previous 4 months will be excluded.

A statistician independent of the project will generate a random allocation sequence prior to commencement of recruitment.
Methods used to generate the sequence in which subjects will be randomised (sequence generation)
The randomised allocation tables will be sent from the statistician to an independent REDCap consultant who will then upload the allocation sequence into the REDCap database. This will ensure all researchers are blinded to the group allocation until after the participant has completed the baseline survey. Participants will be allocated to either the intervention group or to the control group at a ratio of 4:1 respectively with separate randomisation tables for each site.

Regression models will be used to compare the primary outcomes between the health literature and telephone coaching referral conditions at the primary end point of the study: the 6 month follow-up, adjusting for baseline confounding factors such as age, sex, and diagnosis. Similar analyses will be conducted for secondary outcomes.

Summary
Brief summary
Telephone-based behaviour change supports offer a potential avenue for addressing behavioural risks among people with a mental health condition, however, their effectiveness in reducing health risks other than smoking has not yet been explored for this population group. A randomised controlled trial will be conducted to explore the effectiveness of a population-level telephone coaching service (the NSW Get Healthy Information and Coaching Service) in addressing health risk behaviours, including inadequate physical activity and poor nutrition. Compared to a control condition receiving information brochures only, participants exposed to the Get Healthy service are expected to: be engaging in more minutes of physical activity, be consuming more servings of vegetables and fruit, and be more likely to have attempted to change their health risk behaviours.
