Concomittent chronic medical illness,
chronic lung disease other than asthma, drug or alcohol dependence,
inability to attend study visits,
inability to perform spirometry,
>3 courses of oral corticoseroids (OCS) in past year,
Hospital admission for an asthma exacerbation in the past 3 months,
on regular oral prednisolone or theophylline
